What We Do

Since 2013, the Department of Social and Health Services and the Health Care Authority have collaborated on the Health Homes Program to focus intensive care coordination on those Americans with the greatest needs. Care Coordination Organizations (CCO’s) serve that need.

 

Our Mission

Our mission is to help care coordination organizations work better together. We do this by sharing ideas, learning from each other, and advocating for better policies. We want to make sure everyone, especially those with complicated health needs, gets the care they need. We believe everyone should have an equal chance to be healthy and happy, no matter who they are.

Our Vision

A world where health equity is celebrated through whole person health, fulfilled on and led by care coordinators serving the needs of every qualified individual in our State.

More About Health Homes

The goal of the Health Home Program is to improve coordination of care, quality, and to increase an individual’s participation in their own care. Health Homes seek to address complex health issues by offering:

  • comprehensive care management

  • care coordination

  • health promotion

  • comprehensive traditional care and follow up

  • individual and family support

  • referrals for community and social services support
